Job summary

Enhabit, Inc. is seeking a scheduling coordinator in Alabama to arrange patient appointments for field clinicians and ensure seamless care coordination. You will liaise with patients, staff, physicians, and caregivers to maintain high-quality service.

Applicants should have a nursing license (LPN preferred), CPR, and at least 1 year of clinical experience. In-office attendance is essential to meet Enhabit objectives.

Qualifications

• High School Diploma or equivalent required.
• LPN license/clinical license required.
• CPR certification required.
• Minimum 1 year clinical experience in healthcare.
• Home health/hospice/pediatric experience preferred within last 24 months.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills including mobile devices, MS Office.

Responsibilities

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referrals, caregivers and other providers to ensure continuity of care.
• Manage on-call notebook and hospitalization logs to improve communication among stakeholders.
