Job summary

Wellstar Health System in Georgia seeks a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to meet discharge goals.

The CC RN will collaborate with the PAS, financial counselor and UM nurse to ensure appropriate insurance coverage and resources, participate in interdisciplinary rounds, and support efficient, patient-centered discharge planning.

Qualifications

• RN required with acute care nursing experience.
• Excellent written and verbal communication.
• Strong assessment, interview and organizational skills.
• Ability to work collaboratively with physicians and care teams.

Responsibilities

• Assess transitional care needs and psychosocial risk factors.
• Coordinate discharge planning and timely patient progression.
• Collaborate with physicians, PAS, UM nurse and social work for care plans.
• Document progress notes and maintain accurate records.
• Participate in interdisciplinary rounds and care conferences.
