Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs and coordinate across the continuum. You will engage patients and families to ensure goals are met, manage length of stay, and optimize resource use.

Responsibilities include psychosocial assessment, discharge planning, care progression, and collaboration with physicians and care teams to support timely transitions and high-quality care delivery.

Qualifications

• Excellent written and verbal communication skills.
• Self-directed, able to handle stress in a fast-paced environment.
• Strong assessment, interview, organizational and problem-solving skills.
• Knowledge of local, state and federal regulations.
• Familiarity with community resources and care transitions.

Responsibilities

• Assess chronic disease management and psychosocial needs for discharge planning.
• Collaborate with PAS, financial counselor and UM nurse to assess insurance requirements.
• Coordinate care progression and discharge plans with physicians and care team.
• Document all interactions and ensure records are up-to-date in the EMR.
• Participate in interdisciplinary rounds and post-acute referrals to ensure timely discharge.
