Job summary

Live Well Homecare in Georgia seeks a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the care continuum, and engage patients and families to ensure care goals are met.

In this role you will conduct psychosocial and functional assessments, plan and manage discharge processes, and collaborate with physicians, care teams, and social services to promote timely, efficient patient transitions. Documentation and mentorship are key aspects of this role.

Qualifications

• Requires an Associate's or Bachelor's Nursing degree, RN licensure, BLS certification, and at least 1 year of nursing experience in acute care.
• Must possess excellent communication, assessment, organizational, and problem-solving skills.
• Knowledge of regulations and community resources is essential.

Responsibilities

• Conduct psychosocial and functional assessments; plan and manage discharge processes; facilitate interdisciplinary rounds; address barriers to care and discharge.
• Collaborate with physicians, care teams, and social services to promote timely, efficient patient transitions.
• Document assessments, care plans, and interactions; ensure compliance with policies; support professional development; mentor colleagues when appropriate.
• Perform other duties as assigned.
