Job summary

Qualifications

• Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred.
• RN licensure (state-specific) upon hire.
• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication; ability to work under stress.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care across the continuum with the care team and family.
• Participate in interdisciplinary rounds and care conferences.
• Ensure timely documentation and discharge planning; identify barriers.
