Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to manage transitional care needs and coordinate patient care across the continuum. You will engage with patients and families, plan discharge, and work with the care team to ensure goals are met and resources are utilized efficiently.

The role emphasizes psychosocial assessment, care progression, interdisciplinary rounds, and collaboration with physicians and the UM team to optimize patient outcomes and smooth transitions.

• Knowledge of local, state and federal regulations.

Responsibilities

• Assess psychosocial and care needs and plan discharge.
• Collaborate with care team to establish discharge plans.
• Coordinate with physicians, PAS, UM and care team for timely care.
• Document assessments and plans in medical records.
• Identify barriers and escalate to reduce discharge delays.
