Job summary

• Experience documenting care plans and progress in EMR.

Responsibilities

• Assess psychosocial and transitional care needs and plan discharge.
• Coordinate care across the continuum with the care team and payers.
• Engage patient and family to ensure care needs are met.
• Participate in interdisciplinary rounds and care conferences.
• Document progress and barriers in the medical record.
