Job summary

The Care Coordinator RN at Wellstar Health System, Inc. is responsible for assessing transitional care needs, coordinating across the continuum, and engaging with patient and family to assure care needs are met.

The CC RN plans effectively to meet the patient\'s needs, manage length of stay, and promote efficient resource utilization. Responsibilities include discharge planning, care progression, and collaboration with physicians, the care team, and social work to facilitate timely transitions

Qualifications

• Associates or Diploma in Nursing; Bachelor's preferred.
• RN licensure (US) in good standing; multi-state licensure where applicable.
• BLS or ARC-BLS certification required.
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication; ability to work under pressure.

Responsibilities

• Assess transitional care needs and coordinate across the care continuum.
• Lead discharge planning and care progression to support timely and safe discharge.
• Collaborate with physicians, care team, and social services; participate in rounds and care conferences.
• Document care plans and progress in medical records; monitor utilization and barriers.

Education

Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
