Job summary

Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs, coordinate across the care continuum, and engage with patients and families to ensure needs are met. Qualifications

• Associates degree, diploma in nursing, or Bachelor’s with nursing focus (Bachelor’s preferred).
• RN licensure required; BLS certification required or equivalent.
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication; ability to work under pressure.
• RN licensure (single state or multi-state compact) is required.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage with patient and family to assure care needs are met.
• Collaborate with physicians, UM nurse, PAS, and care team to develop discharge plans.
• Attend interdisciplinary rounds and care conferences to discuss progress and facilitations.
• Document in EMR and monitor avoidable days and discharge milestones.
