Job summary

Wellstar Health System, Inc. is seeking a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. The role involves engaging patients and families to ensure care needs are met and planning effective transitions.

You will collaborate with physicians and the interdisciplinary care team, facilitate discharge planning, participate in rounds and care conferences, and document progress to support timely discharge and resource utilization.

Qualifications

• RN licensure and BLS certification required.
• Minimum 1 year nursing experience in acute care setting.
• Bachelor's degree preferred; associate degree acceptable.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage patients and families to ensure care needs are met.
• Facilitate discharge planning and collaborate with the care team.
• Participate in interdisciplinary rounds and care conferences.
