Qualifications

• Associates in nursing or a diploma (RN) required.
• Bachelor’s in Nursing preferred.
• RN license (Single State) or RN-COMPACT (multi-state).
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication; strong teamwork.

Responsibilities

• Assess transitional care needs and coordinate discharge planning.
• Collaborate with physicians, care team, patient and family.
• Facilitate care progression and post-acute referrals.
• Document in medical record and track discharge milestones.
• Engage in interdisciplinary rounds and patient education.
• Identify barriers to discharge and resolve delays.
