The Care Coordinator RN (CC RN) at Wellstar Health System, Inc. assesses transitional care needs, coordinates care across the continuum, and engages with patients and families to meet discharge goals.

The role plans effectively to manage length of stay and optimize resource use, integrating care progression with discharge planning. Collaborates with utilization review, physicians, and the care team to assess medical and psychosocial needs, remove barriers, and ensure timely discharge;

Qualifications

• RN license required or multi-state compact (as applicable)
• Minimum 1 year nursing experience in acute care
• BLS/ACLS certification required upon hire

Responsabilités

• Initiates assessment of chronic disease management and psychosocial risks at admission.
• Collaborates with PAS, financial counselor and UM nurse to assess insurance needs for payers.
• Develops discharge plans with patient, family, and care team to ensure timely discharge.
• Meets with physicians and care team to coordinate care progression and discharge planning.
• Manages all aspects of discharge planning and resources for each patient.
• Documents care progression and barriers in the medical record to ensure continuity of care.
• Coordinates post-acute referrals and connects patients with community resources.
