Job summary

Wellstar Health System, Inc. is seeking a Care Coordinator RN to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure care needs are met. The role focuses on discharge planning, resource utilization, and care progression.

Responsibilities include psychosocial assessment, discharge planning, interdisciplinary rounds, documentation, and collaboration with physicians, the PAS, UM, and care team to support timely discharge and smooth transitions.

Qualifications

• Active RN license (single state) or multi-state compact licensure.
• Minimum 1 year nursing experience in the acute care setting.
• BLS certification required upon hire.
• Excellent communication and ability to work under pressure.

Responsibilities

• Assess transitional care needs and plan discharge with patient and family.
• Coordinate care across continuum with the care team.
• Participate in interdisciplinary rounds and family education.
• Collaborate with physicians, case management, and UM to support discharge goals.
