Wellstar Health System, Inc. is seeking a Care Coordinator RN (CC RN) to assess transitional care needs and coordinate care across the continuum.

The role emphasizes engaging with patients and families to meet care needs, manage length of stay, and optimize resource use. The CC RN will participate in care conferences, rounds, and education, working with the utilization review nurse and care team to support discharge planning and progression to the next level of care.

Qualificações

• Associates degree, diploma, or Baccalaureate-level nursing program.
• RN licensure and BLS certification required at hire.
• Strong communication and organizational skills essential.
• Experience in acute care nursing preferred.

Responsabilidades

• Assess psychosocial status and transitional care needs.
• Coordinate care across continuum and discharge planning.
• Collaborate with physicians, care team, patient and family.
• Document in EMR and track progress toward discharge goals.
