Job summary

Heritage Behavioral Health Center in Decatur, IL, is seeking a Program Manager - Care Coordination to oversee daily operations and ensure high-quality, person-centered care coordination services. You will supervise Care Coordinators, strengthen multidisciplinary collaboration, and drive program improvement using data and outcomes.

This full-time role requires a Bachelor's degree and experience in behavioral health leadership; strong knowledge of Medicaid/Medicare and regulatory requirements is

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or related field.
• Valid driver’s license and reliable transportation.
• Experience supervising residential or community-based behavioral health programs preferred.

Responsibilities

• Provide leadership, coaching, mentorship, and support to Care Coordinators.
• Oversee timely service linkages, follow-up, and care coordination for individuals with complex needs.
• Develop and manage care coordination workflows and initiatives; monitor quality and outcomes.
• Collaborate with clinical leadership and internal service lines to align strategies.
• Ensure documentation meets regulatory and payer requirements; use EHR and case management systems.
