Heritage Behavioral Health Center in Decatur, Illinois is seeking a Care Coordinator to provide person-centered care coordination, ensuring individuals receiving behavioral health services receive seamless, comprehensive, and ongoing support within their assigned program area.

This role tracks clients from initial engagement through successful connection to all recommended treatments, collaborating with primary care teams and community resources to ensure timely follow-up and adherence to

Qualifications

• High School Diploma or equivalent plus 5 years of supervised mental health or social service experience
• Bachelor’s degree in social work, psychology, education, or human service field preferred
• Minimum of 21 years of age
• A valid driver’s license, reliable means of transportation, and proof of automobile insurance

Responsibilities

• Coordinate connections to behavioral health, primary, and specialty care services
• Follow up with clients to monitor progress and address barriers
• Collaborate with care teams and community partners to arrange appointments
• Educate clients and families on continuity of care and available resources
• Assist clients in navigating healthcare and social service systems
• Participate in interdisciplinary team meetings to discuss progress
• Advocate for client needs and empower self-determination
