Job summary

Program Manager - Care Coordination

Salary: Range based on education, experience, and licensure or certification

  • Bachelor’s degree: $60,000 annually + every other Friday off paid
  • Master’s degree: $65,000 annually + every other Friday off paid
  • Master’s degree + LPC, LSW, or CADC: $67,000 annually + every other Friday off paid
  • Master’s degree + LCSW, LCPC, or LMFT: $70,000 annually + every other Friday off paid

Schedule: Full-Time | Every other Friday off (paid wellness day)

Your Role

Program Manager - Care Coordination

The Program Manager of Care Coordination oversees the daily operations, quality, and effectiveness of care coordination services within our agency. This leader directly supervises Care Coordinators and helps ensure that individuals with complex behavioral health needs receive timely, comprehensive, and person-centered support.

The Program Manager leads and strengthens multidisciplinary collaboration, guides program improvement, and uses data to advance quality, efficiency, client safety, and health equity. The role also develops strong partnerships with internal service lines, healthcare providers, social service agencies, referral sources, patients, and families.

Core Responsibilities

Supervision and Leadership

  • Provide leadership, coaching, mentorship, and support to Care Coordinators.
  • Conduct regular team meetings, performance evaluations, and professional development activities.
  • Ensure adherence to best practices, agency policies, and person-centered care standards.
  • Promote a culturally responsive, collaborative, and supportive team environment.

Care Coordination Oversight

  • Oversee timely service linkages, follow-up, and care coordination for individuals with complex needs.
  • Facilitate multidisciplinary collaboration to address gaps in care and reduce fragmentation.

Program Development and Quality Improvement

  • Develop and manage care coordination workflows and initiatives aligned with organizational priorities.
  • Use data, performance indicators, and outcomes to evaluate effectiveness and identify improvement opportunities.
  • Monitor regulatory compliance, quality standards, and fidelity to care coordination models.
  • Manage program resources, budgets, and timelines to support implementation and sustainability.
  • Maintain required program reports, metrics, and documentation.

Collaboration and Partnerships

  • Collaborate with clinical leadership and internal service lines to align care coordination strategies with broader health system goals.
  • Build strong relationships with healthcare systems, community resources, social service agencies, and referral partners.
  • Support clear cross-department communication and integrated care efforts.

Documentation and Compliance

  • Ensure documentation meets agency, state, federal, and payer requirements.
  • Promote accurate, timely documentation and effective use of electronic health record and case management systems.
  • Adapt program practices to changing regulations, service needs, and organizational demands.

Knowledge, Skills, and Abilities

Knowledge of

  • Care coordination and case management models
  • High-risk intervention strategies and crisis response
  • Behavioral health, substance use, and co-occurring disorders
  • Social determinants of health and their impact on wellness
  • Medicaid, Medicare, healthcare reimbursement, and applicable regulatory requirements
  • Community resources and systems serving individuals with complex needs

  • Leadership, supervision, coaching, and staff development
  • Crisis intervention, problem-solving, and multidisciplinary collaboration
  • Program evaluation, data analysis, performance measurement, and quality improvement

Education and Experience

Required Qualifications

  • Bachelor’s degree in social work, psychology, counseling, human services, or a related field required.
  • Valid driver’s license, reliable transportation, and proof of automobile insurance required.
  • Experience supervising residential or community-based behavioral health programs preferred.
