Qualifications

• Bachelor's degree in social work, psychology, counseling, human services, or a related field required.
• Valid driver's license, reliable transportation, and proof of automobile insurance required.
• Experience supervising residential or community-based behavioral health programs preferred.

Responsibilities

• Provide leadership, coaching, mentorship, and support to Care Coordinators.
• Oversee timely service linkages, follow-up, and care coordination for individuals with complex needs.
• Develop and manage care coordination workflows aligned with organizational priorities.
• Collaborate with clinical leadership and internal service lines to align strategies.
• Ensure documentation meets agency, state, federal, and payer requirements.
