Heritage Behavioral Health Center- HBHC in Decatur, Illinois is seeking a Program Manager of Care Coordination to oversee daily operations, improve quality, and ensure person-centered support for individuals with complex behavioral health needs.

You will supervise Care Coordinators, lead multidisciplinary teams, develop workflows, monitor metrics, ensure regulatory and payer compliance, manage budgets, and build partnerships with healthcare providers and social service agencies to sustain

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or related field is required.
• Valid driver’s license and reliable transportation required.
• Experience supervising residential or community-based behavioral health programs preferred.

Responsibilities

• Provide leadership, coaching, and supervision to Care Coordinators.
• Oversee care coordination workflow, timeliness, and outcomes.
• Promote multidisciplinary collaboration and partnerships.
• Ensure documentation meets regulatory and payer requirements.
• Monitor program quality, performance indicators, and fidelity to models.
• Manage program resources, budgets, and timelines.
