Job summary

Limitless Decatur at the Community Health Improvement Center in Decatur, IL is seeking a Care Coordinator in a full-time role. Qualifications

• Must hold a Certified Medical Assistant credential.
• Health education experience preferred.
• Healthcare office experience is required.

Responsibilities

• Process referrals/diagnostics/prior authorizations within outlined workflows.
• Ensure therapeutic relationship with patients to encourage active participation in their care.
• Maintain referral site spreadsheet and coordinate follow-up.
• Obtain prior authorizations as needed and access relevant portals.
• Schedule diagnostics and complete ConferMed electronic referrals.
• Process monthly DMH lung screenings.
• Serve as a liaison with local hospitals, insurers, and specialist offices.
• Other duties as assigned.
