This full-time leadership role oversees a Community Support Services team delivering recovery-focused, intensive community-based rehabilitation for adults with mental health and co-occurring disorders. The role combines clinical practice with administrative supervision, providing services across on-site and off-site locations, tracking quality measures, and coordinating care to support

Qualifications

• Master’s degree in counseling, social work, psychology, or related field.
• Licensure as CADC/LSW/LCSW/LPC/LCPC is highly recommended.
• Ideal candidates have experience with mental health or substance use diagnoses.

Responsibilities

• Provide team and individual supervision.
• Monitor quality measures and standards.
• Deliver off-site services aligned with individual service plans.
• Complete assessments like IATP and LOCUS with required documentation.
• Develop crisis and treatment plans in collaboration with clients and team.
• Coordinate referrals to ensure continuity of care.
• Promote reduction of crisis episodes and improved community functioning.
• Participate in bi-weekly team meetings and stay informed on clients' progress.
