Qualifications

• Certification per American Association of Medical Assistants preferred (AAMA) or LNA license.
• BLS certification and proficient computer skills required.
• Experience working with an electronic health record preferred.

Responsibilities

• Support value-based care efforts within family practice and coordinate patient care in a PCMH.
• Collaborate in a team-based care model to improve patient outcomes and close care gaps.
