Job summary

Grace Cottage Hospital in Vermont seeks a Care Coordinator to support value-based care efforts within family practice. This cross-functional role focuses on improving patient outcomes, closing care gaps, and ensuring payer incentive program compliance while coordinating patient care within the Grace Cottage Family Health (GCFH) PCMH.

The coordinator uses data analytics and patient outreach to proactively manage patient populations and support quality metrics that align with financial incentives

Qualifications

• Certification or licensure is preferred for MA/LNA.
• BLS certification required; proficient computer skills.
• Experience with electronic health records is preferred.

Education/ Experience: Certification per American Association of Medical Assistants preferred (AHA) or LNA License.

Other Requirements: BLS, Proficient computer skills required. Experience working with an electronic health record preferred.
