Job summary

ProMedica Physician Group, Inc. is seeking a Quality Care Coordinator to advance value-based care initiatives, close care gaps, and improve patient outcomes.

You will act as a liaison among patients, families, clinicians, and care teams to ensure coordinated, efficient care aligned with quality goals. The role includes outreach for preventive care, follow-up coordination, and identifying barriers to access, while applying clinical quality data to find and close opportunities within the health

Qualifications

• High school diploma or equivalent.
• 3 years of experience in a physician’s office or health care setting.
• Willing to obtain CMA/RMA and CPR certifications within 90 days.

Responsibilities

• Coordinate care and follow-up to advance value-based care initiatives.
• Identify barriers to access and engage patients and families.
• Abstract data and enter information into the EHR; generate reports.
