Job summary

Grace Health in Battle Creek, MI is seeking a Quality Coordinator to help coordinate quality initiatives and monitor performance against standards. You will support data-driven improvements and collaborate with care teams to enhance patient outcomes and the overall quality of Grace Health’s services.

The role emphasizes analyzing quality metrics, supporting process improvements, and ensuring accurate documentation in EMR. A healthcare background and strong communication skills are preferred.

Qualifications

  • Advanced work-related skills beyond completion of high school, including written and verbal communications, computer skills, mathematical, technical or health care related knowledge.
  • Knowledge and training in the field of healthcare.
  • Expected to learn and maintain higher level skills of the trade/profession, in addition to the basic training of the trade/profession.

Responsibilities

  • Possesses a comprehensive understanding of health plans, quality care frameworks and related measures (e.g. HEDIS).
  • Monitors quality metrics, category changes, volume/census data, and reporting requirements to ensure compliance.
  • Researches to identify best practices and opportunities for improving quality outcomes.
  • Provides actionable recommendations to enhance performance in key focus areas.
  • Analyzes reports to identify patient engagement methods and ensures documentation in EMR.
  • Leverages advanced technologies to evaluate, monitor and report data to drive improvements in patient utilization of Grace Health’s services.
  • Reviews patient records to evaluate quality indicators and address gaps in care.
  • Develops and tracks improvement strategies targeting low-performing indicators with measurable progress.
  • Supports quality team in implementing initiatives to enhance patient care outcomes.
  • Assists in developing and refining processes and procedures to support organizational change.
  • Collaborates with cross-functional teams to drive quality improvement across the agency.

  • - Possesses a comprehensive understanding of individual health plans, quality care frameworks, and associated measures (e.g. healthcare effectiveness data and information set (HEDIS).
  • - Monitors quality metrics, category changes, volume/census date, and reporting requirements to ensure compliance and effective implementation.
  • - Conducts research to identify best practices and opportunities for improving quality outcomes.
  • - Provides actionable recommendations and insights to enhance performance in key focus areas.
  • - Analyzes written and electronic reports to identify appropriate patient engagement methods (e.g., mail, phone) and ensures accurate documentation in the electronic medical record (EMR).

  • - Reviews and assesses patient medical records to evaluate quality indicators and address gaps in care.
  • - Develops and tracks improvement strategies targeting low-performing indicators, ensuring measurable and sustainable progress over time.
  • - Supports the quality team to implement initiatives aimed at enhancing patient care outcomes.
  • - Supports organizational change by assisting in the development and refinement of processes and procedures.
  • - Collaborates with cross-functional teams to drive quality improvement efforts across the agency.
Professional/Technical Knowledge:
  • 1) Advanced work-related skills beyond completion of high school, including written and verbal communications skills, computer skills, mathematical, technical or health care related knowledge frequently acquired through work experience or an associate degree program.
  • 2) Knowledge and training in the field of healthcare
