Job summary

HealthierHere in Seattle seeks a Care Coordination Operations Manager to oversee the Community Hub’s workflows and policies, driving scalable, compliant operations. You will own CBCC processes, care transitions, and HRSN workflows, coordinating with the Connect2 technology team and funders.

You will supervise a small team, onboard staff, and translate grants into sustainable operations while partnering with multiple teams to improve service quality and performance.

Qualifications

• 5+ years of experience within community health, public health, and/or related fields, with 2 years in program management.
• Experience designing clinical or community based organization program workflows.
• Problem‑solving and troubleshooting, excellent writing and communication skills.
• Strong organizational and interpersonal skills, time management, attention to detail, and follow through.
• Experience using Microsoft Office, Word, Excel, SharePoint, PowerPoint, and other relevant computer programs to develop materials, monitor and communicate progress.
• Experience supervising, coaching, and developing staff in a multidisciplinary environment.
• Demonstrated ability to build, and maintain, trusted relationships with communities served by HealthierHere’s programs and initiatives.

Responsibilities

• Lead the HealthierHere Community Hub operations and workflows for CBCC, HRSNs, and care transitions.
• Develop policies, SOPs, and procedures to ensure compliant, scalable service delivery.
• Onboard and offboard Community-Based Workers and supervise staff as needed.
• Collaborate to translate grants and funder requirements into sustainable operations.
• Analyze performance dashboards and drive improvements with cross‑functional partners.
