Job summary

St. Joseph's Hospital & Medical Center in Phoenix seeks a Care Coordination Nurse to lead discharge planning for a diverse inpatient population from birth to adult.

You will orchestrate care delivery, advocate for patients, and address social factors to ensure safe, cost-effective transitions of care. You will conduct assessments, coordinate multidisciplinary teams, and oversee plan implementation to optimize resource use and patient outcomes.

Qualifications

• Two years acute hospital clinical experience or a Master's in Case Management or Nursing in lieu of one year.
• Graduate of an accredited school of nursing.
• Registered Nurse: AZ or Compact license.
• American Heart Association BLS within 90 days.
• Knowledge of managed care and payer environment.
• Must have critical thinking and problem-solving skills.
• Collaborate effectively with multiple stakeholders.
• Understand how utilization management and case management programs integrate.
• Ability to work as a team player and assist other members of the team where needed.
• Knowledge of CMS standards and requirements.

Responsibilities

• Completes and documents discharge planning assessments for patients identified by the screening process.
• Reassess the patient and update the plan as appropriate.
• Facilitate development of a multidisciplinary discharge plan with relevant team members and post-acute providers.
• Oversee and evaluate the implementation of the discharge plan.
• Collaborate with the care team to ensure progression of care and appropriate inpatient resource use.
• Coordinate with post-acute providers to ensure timely transitions to appropriate services.
