Job summary

CommonSpirit Health is seeking an RN Care Coordinator to oversee care progression and discharge planning for identified patients. You will collaborate with utilization management, nursing, physicians, and post-acute providers to ensure transitions align with patient needs and preferences.

The role emphasizes communication, advocacy for patients and families, and adherence to regulatory standards. Minimum CA RN license and BLS are required; advanced certifications are preferred.

Qualifications

• Minimum two (2) years of acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of 1 year experience.
• Current and active California RN license
• AHA BLS card

Responsibilities

• Oversee discharge planning assessment and update the plan as needed.
• Develop multidisciplinary discharge plans involving the care team and post-acute providers.
• Oversee and evaluate the implementation of the discharge plan.
• Coordinate with healthcare teams and post-acute providers for timely transitions.
• Deliver patient notifications and related documentation.
• Participate in performance improvement teams as necessary.
