Job summary

Qualifications

• Bachelor's Degree in Nursing or Master's in Social Work (or equivalent education/experience).
• Minimum of three years in clinical care coordination or three years as an acute nursing team lead.
• Current RN license in California (no license required for MSW).
• CCM or ACM certification within 1 year of hire.
• Experience with electronic health records and discharge planning.

Responsibilities

• Provide operational leadership for Care Coordination through supervision, coaching, and performance management.
• Coordinate and implement Care Coordination programs to support patient progression and outcomes.
• Manage daily operations: staffing, workload balancing, scheduling, productivity, and resource allocation.
• Supervise front-line Care Coordination staff including case managers and social workers.
• Ensure timely completion of patient assessments, discharge planning, and care transitions per standards.
