Job summary

Luminis Health, Inc. is seeking an Advanced Medical Assistant to support the Advanced Primary Care Team in coordinating patient care, outreach, education, and barriers to treatment goals.

You will work to ensure patients receive the right care at the right time to reduce unnecessary ER visits and readmissions. You will handle administrative and clinical tasks, maintain knowledge of care coordination tools, and collaborate with providers to facilitate timely follow-up and effective communication

Qualifications

• Minimum three years of experience as a Medical Assistant.
• Completion of an accredited medical assistant training program preferred.
• Strong customer service skills and basic math skills.

Responsibilities

• Coordinate patient care within the Advanced Primary Care Team and value-based contracts.
• Provide patient outreach and education to engage treatment plans.
• Pre-visit planning with the LPN Team or provider.
• Support data capture and reporting for quality metrics.
