Job summary

Upperline Health is seeking a Primary Care Medical Assistant to deliver high-quality, patient-centered care within a value-based practice.

In this role, you will support patients in the clinic by rooming, taking vital signs, obtaining medical history, and coordinating care beyond visits. You will assist the Primary Care Provider to ensure efficient operations and collaborative team-based care.

Qualifications

• Certified Medical Assistant certification or minimum of two years in primary care
• Experience in outpatient clinic settings strongly preferred
• Strong knowledge of medical terminology and basic pharmacology
• Experience with team-based care models and care coordination

Responsibilities

• Room patients, obtain and document vital signs, medical history, and chief complaints
• Assist providers with in-clinic visits and health assessments
• Support virtual provider encounters from the clinic setting
• Document clinical data in the electronic health record (EHR) accurately
• Perform medication reconciliation and identify discrepancies
• Coordinate preventive and ongoing care through telephonic outreach
• Manage in-basket messages, prescription refills, and prior authorizations
• Support referrals, scheduling, records, and DME coordination
• Assist patients with accessing community resources
• Prepare and maintain clinical equipment and supplies
• Collaborate with interdisciplinary team members
• Participate in team huddles and meetings
• Perform clinical tasks within scope (injections, phlebotomy, POCT)
• Perform other duties as assigned
