Job summary

luminishealth is seeking an Advanced Medical Assistant to support the primary care team by coordinating patient care, conducting targeted patient outreach, and delivering education to improve treatment adherence. In this role you will assist with pre-visit planning alongside LPNs and providers, manage care coordination across services, and use EPIC registries and CRISP portals to pull and attest metrics.

Strong customer service, basic math, and MS Office skills are essential.

Qualifications

• Minimum of High School Diploma
• Completion of accredited medical assistant training program preferred
• Three years of experience as a Medical Assistant
• Experience with Microsoft Office and Excel
• Strong customer service skills
• Basic math skills.

Responsibilities

• Coordinate patient care and empanelment with care teams.
• Provide patient outreach and education to support treatment goals.
• Support pre-visit planning and follow-up activities with LPNs and providers.
• Assist in scheduling, referrals, and coordinating care across services.
• Maintain access to Value Based Payor portals and report metrics.
