Job summary

Grady Health System in Atlanta, GA is seeking a Clinical Care Coordinator to proactively identify barriers to patient flow and coordinate care across emergency, surgical, nursing, and support services. The role ensures appropriate bed placement, discharge planning, and utilization of resources to meet patient needs.

The position requires a nursing background with at least 2 years of RN experience; Nurse Case Management is preferred.

Qualifications

• Bachelor's degree in nursing (BSN) required.
• Minimum 2 years of RN experience.
• Nurse Case Management experience preferred.

Responsibilities

• Act as liaison between emergency and surgical services, nursing, and other teams to optimize patient flow.
• Identify barriers to discharge and coordinate care plans with physicians and care teams.
• Review admissions and status to validate appropriate level of care.
• Coordinate patient progression with MSW and nursing and communicate updates to care team.
• Provide education to staff and patients on care plans and discharge goals.
