Job summary

CHRISTUS Health is seeking a Care Manager (PRN) to collaborate with patients, families, physicians, and a multidisciplinary team from admission through discharge. The CM enhances patient flow, satisfaction, and safety while ensuring regulatory compliance and effective transitions across care settings.

The role emphasizes comprehensive assessment, planning, implementation, and evaluation of needs, with focus on discharge planning and coordination of post-acute resources.

Qualifications

• Certificate, Associate, or bachelor’s degree in nursing.
• Bachelor’s or Master’s degree in Social Work.
• Experience in clinical or acute care setting preferred.
• RN/LBSW/LMSW/LCSW licenses required in state of employment; BLS preferred.

Responsibilities

• Interviews patients/families to assess social, emotional, and financial factors affecting discharge.
• Collaborates with CMII/CMIII to develop and monitor plan of care.
• Coordinates patient care progression across continuum for timely discharge.
• Advocates for patient needs and coordinates post-acute care options.
• Documents care plans and communicates updates to the care team.
