Job summary

Penn State Health - Hershey Medical Center is seeking a Manager of Care Coordination to lead case management and social work teams dedicated to improving patient throughput and care transitions. The role directs discharge planning, resource utilization, and interdepartmental collaboration across care settings.

Qualifications

• Master's of Social Work Degree or Bachelor's Degree in Nursing required.
• 5 years experience in acute care required.
• Progressive leadership experience required.
• Either: (A) currently licensed to practice as a registered nurse by Pennsylvania Board of Nursing required, OR (B) currently licensed as a social worker by Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors required.

Responsibilities

• Manage daily operations of case management and social work teams.
• Coordinate discharge planning and community resource utilization for patients.
• Oversee transfers between acute care, rehab, nursing homes, home care, etc.
• Collaborate with insurance companies, nursing staff, patients, families, and physicians to optimize care delivery.
