Qualifications

• BSN required upon hire
• MSW required if not a Registered Nurse upon hire
• Minimum 3 years of clinical care coordination or 3 years of acute nursing leadership experience
• Operational healthcare experience and staff development
• Licensed for MSW
• RN license in CA
• Certified Case Manager or Accredited Case Manager within 12 months

Responsibilities

• Provide operational leadership for Care Coordination through supervision, coaching, and performance management.
• Oversee daily department operations: staffing, scheduling, productivity, and resource allocation.
• Ensure timely patient assessments, discharge planning, utilization management, and care transition activities in line with standards.
• Monitor performance metrics, identify trends, and drive corrective actions and improvements.
• Lead multidisciplinary rounds and partner with clinicians to resolve operational issues across care continuum.
• Support accreditation readiness and regulatory compliance through documentation and processes.
