Job summary

CNS Cares, LLC is seeking a Care Manager to oversee the delivery of coordinated care for an assigned group of patients and to schedule direct care staff for visits. The role focuses on maximizing authorization units, advocating for patient needs, and ensuring quick, accurate service with exceptional patient satisfaction.

The position requires communicating with patients, nurses, and the claims team, using EMR to schedule visits, source staff, and maintain clinical compliance.

Qualifications

• Requires High School Diploma and 2+ years in healthcare.
• Needs at least 2 years of staff scheduling experience.
• Must be at least 21 years old.
• Associate's or Bachelor's degree preferred.

Responsibilities

• Demonstrate regular attendance and punctuality; collaborate with all levels of staff.
• Schedule patient visits in the EMR and secure coverage as needed.
• Source and hire staff with local leadership and recruiting teams.
• Ensure all authorized units are staffed and overtime is minimized.
• Coordinate with DME for patient equipment needs and ensure timely delivery.
• Advocate for appropriate care levels during renewals and start of cares.
• Attend medical appointments to support physicians on DOL Home Health Care program matters.
• Gather medical records as needed.
• Maintain compliance with company and state guidelines.
• Communicate with patients, nurses, home health aides, and claims team regularly.
• Participate in on-call rotation and adhere to policies.
• Perform other duties as assigned.
