Job summary

Caring for Others is seeking a Care Manager to serve as a central hub for care coordination, scheduling, and client communication within our in-home care team.

You will coordinate services, develop individualized plans, manage authorizations for billing, and support branch compliance while leading caregiver onboarding.

This role emphasizes HIPAA compliance, strong documentation, and a warm, professional approach to families, clients, and referral partners.

Qualifications

• Experience in home care, healthcare coordination, or a client-facing service role preferred.
• Highly organized with strong attention to detail and accurate record-keeping practices.
• Proactive communicator with interpersonal skills and professional presence.
• Solutions-oriented and able to handle multiple priorities in a fast-paced environment.
• Committed to compliance and maintaining accurate documentation.
• Strong collaboration with a team-first mindset and customer service approach.
• Proficient with healthcare management or scheduling systems.
• Experience in assisted living, in-home care, or billing/authorization management is a bonus.

Responsibilities

• Provide 5-star service to Clients, families, Caregivers, and partners.
• Coordinate care services for timely, consistent delivery for all Clients.
• Develop individualized service plans and conduct initial and ongoing Client assessments for quality.
• Prepare Client materials and manage authorizations to support billing accuracy.
• Support scheduling and staffing to minimize overtime and ensure coverage.
• Provide after-hours support to maintain seamless care continuity.
• Maintain Branch compliance and licensing documentation.
• Conduct weekly New Hire Orientation sessions for incoming Caregivers.
