The role involves coordinating daily provider operations, patient tracking, and documentation, with a focus on transitions of care and scheduling efficiency.

Qualifications

• Associate's degree required or bachelor’s in healthcare administration or equivalent experience.
• 2–5 years of healthcare operations, care coordination, or hospital administration.
• Experience with EMRs, scheduling systems, and Microsoft Office.
• Excellent verbal and written communication abilities.

Responsibilities

• Coordinate and maintain provider coverage schedules and meetings.
• Track patient census and assign providers per payer relationships and guidelines.
• Facilitate patient handoffs to FastPAC and post-acute care teams.
• Attend interdisciplinary rounds and communicate updates to stakeholders.
• Support billing documentation, payroll submission, and performance reporting.
