Job summary

CVS Health in Louisiana is seeking a full-time Care Management professional to coordinate care, support members, and educate them using care management resources to improve outcomes.

You will collaborate with case managers, medical directors, and health programs, identify barriers, and apply practical case management knowledge to implement policies and programs. Strong communication and ability to travel are essential.

Qualifications

• Degree in Health Care/Human Services related field is required.
• Experience in Case Management is required.
• Strong customer service and problem solving skills.
• Excellent communication skills, including telephonic.
• Motivational interviewing skills are preferred.
• Strong documentation and regulatory compliance.
• Highly organized with time-management skills.
• Ability to travel within 125 miles round trip for 1 day.
• Flexible schedule including evenings up to 9pm.

Responsibilities

• Consult with case managers and health program staff to overcome barriers.
• Present cases at conferences for multidisciplinary review.
• Identify and escalate quality of care issues.
• Apply negotiation to secure appropriate options and services.
• Use motivational interviewing to engage members and promote healthy changes.
• Provide coaching and information to empower members.
• Encourage member participation with providers in decision making.
• Evaluate referred members' needs and determine case resolution.
