Qualifications

• Degree in Health Care/Human Services related field.
• Case Management Experience.
• Strong customer service orientation and problem solving skills.
• Excellent communication/telephonic skills.
• Excellent motivational interviewing skills/rapport building telephonically.
• Excellence in documentation and compliance/regulatory standards.
• Highly organized, self-driven, and able to work with minimal supervision.
• Time management and ability to meet performance metrics.
• Technology proficiency with typical healthcare platforms.
• Willingness to travel within 125 miles for a 1-day field assignment.
• Flexible schedule including some evenings (up to 9pm local time) and 2 days weekly.

Responsibilities

• Consult with case managers, supervisors, medical directors and other health programs to overcome barriers.
• Present cases at conferences to obtain multidisciplinary review for optimal outcomes.
• Identify and escalate quality of care issues through established channels.
• Demonstrate negotiation skills to secure appropriate options and services.
• Deliver coaching and motivational interviewing to promote healthy choices.
• Provide information and support to empower members in medical decisions.
• Encourage member participation with providers in decision-making.
• Conduct comprehensive evaluations of referred members and recommend approaches.
