Job summary

CVS Health is seeking a Care Management professional in Louisiana to coordinate care, educate members, and support case management initiatives. The role focuses on applying practical care-management knowledge to policies and processes across the program.

You will work with case managers, medical directors, and health programs to improve outcomes, with emphasis on telephonic communication and member engagement. This full-time position offers comprehensive benefits and requires regional travel.

Qualifications

• Minimum requirements include a health care/human services degree and case management experience.
• Need strong customer service orientation and problem solving.
• Excellent communication and telephonic skills; strong documentation/ compliance.
• Highly organized, self-driven with time management and reliability.
• Travel up to 125 miles for 1 day in the field; flexible evening hours may be required.

Responsibilities

• Consult with case managers and medical directors to overcome barriers and achieve goals.
• Present cases at case conferences for multidisciplinary review.
• Identify and escalate quality-of-care issues through established channels.
• Demonstrate negotiation skills to secure appropriate options and services for members.
• Deliver motivational interviewing to engage members in healthy choices.
• Provide coaching and information to empower members in medical decisions.
