The role involves outbound and inbound calls with members and healthcare partners, building compassionate relationships, and supporting Tailored Care Managers by coordinating services and documenting interactions. A background in healthcare or customer service is a plus.

Qualifications

Responsibilities

• Make outbound and receive inbound calls with members and healthcare partners.

• Assist Tailored Care Managers by completing outreach, scheduling appointments, coordinating services, and documenting member interactions.
• Help connect members with healthcare providers, community resources, and essential support services.
• Maintain accurate documentation while meeting quality and productivity goals.
