Pathways to Life, Inc. is seeking a Tailored Care Manager Extender to serve as a friendly voice on the phone, connecting members with care, services, and support to help them live healthier lives.

The role involves outbound and inbound calls with members and healthcare partners, building compassionate relationships, and supporting Tailored Care Managers by coordinating services and documenting interactions. A background in healthcare or customer service is a plus.
