CVS Health is seeking a care management professional in Louisiana to coordinate, support and educate members using care management resources to improve health outcomes. The role involves implementing projects and processes for Case Management and applying practical knowledge to administer policies and plans for the area.

The position requires 2+ years of experience in behavioral health or related fields, ability to travel, and willingness to work evening hours.

Qualifications

• 2+ years' experience in behavioral health, social services or appropriate related field equivalent to program focus.
• 2+ years' experience with adult and/or adolescent populations.
• Must reside in Louisiana - Region 4 (Parishes: Evangeline, St. Landry, Acadia, Lafayette, St. Martin, Vermillion, or Iberia)
• Ability to travel up to 50% of time.
• Ability to interface with customers/clients.
• Ability to work 2 days weekly until 9pm local time.

Responsibilities

• Consults with case managers, supervisors, medical directors and/or other health programs using a holistic approach to overcome barriers to meeting goals and objectives.
• Presents cases at case conferences to obtain a multidisciplinary review to achieve optimal outcomes.
• Identifies and escalates quality of care issues through established channels.
• Demonstrates negotiation skills to secure appropriate options and services to meet the member's benefits and healthcare needs.
• Delivers influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum health.
• Provides coaching, information, and support to empower the member to make ongoing independent medical and healthy lifestyle choices.
