Job summary

Norman Regional Health System seeks a Care Coordination Health Partner to support care coordination across clinics and provider teams. You will collaborate with care coordinators, providers, and community services to ensure smooth patient navigation and effective care plans.

Responsibilities include advocating for patients, communicating with families and partners, and applying current best practices to improve outcomes.

Qualifications

• Customer service experience preferred.
• Bilingual preferred but not required; basic computer skills.
• Excellent oral and written communication; highly organized and self-motivated.

Responsibilities

• Assist with coordination of care with multiple external agencies.
• Act as patient advocate and liaison among patient, caregivers, and healthcare team.
• Stay updated on trends in the field and support quality improvement initiatives.
• Utilize motivational interviewing and culturally sensitive methods with patients.
• Maintain high communication with Care Coordinators and clinical team to support ongoing care coordination.
