Job summary

Norman Regional Health System seeks a Care Coordination Health Partner to support coordination of care with multiple external agencies and provide enhanced health promotion and disease management for patients.

You will work closely with clinic-based staff, acute care staff, and post-acute providers to ensure continuity of care and navigate healthcare needs for patients in clinic and by phone conference.

Qualifications

• Customer service experience preferred.
• Bilingual preferred but not required; basic computer skills and strong communication.
• Excellent written and oral communication; highly organized and self-motivated.

Responsibilities

• Collaborate with Care Coordinators and Providers to coordinate care across multiple external agencies.
• Act as patient advocate and liaison among patient, caregivers, healthcare team, and community services.
• Keep current with trends in care coordination and integrate into practice under leadership.
• Use motivational interviewing and culturally sensitive methods with patients.
• Maintain communication with Care Coordinators and clinical team to support ongoing care needs.

• $21.15-$34.55/hr. depending on previous work experience

This department is where successful use of data and evidence based nursing care meets quality improvement, and practice transformation. Our department includes Clinic Care Coordinators who work directly with Norman Regional Clinics and their patients to provide an enhanced level of health promotion, care coordination, and disease management. We enjoy a collaborative process with both clinic based staff, acute care staff, and post-acute providers to ensure continuity of care, so our patients can truly consider Norman Regional Health System their medical home. The Care Coordination Health Partner see's patients in the clinic and/or via phone conference to assist patients with health care navigation needs.

Job Summary

• Collaborates with Care Coordinators as well as Providers to assist with coordination of care with multiple external agencies to best serve the needs of the patient.
• Act as a patient advocate and liaison between the patient, caregivers, healthcare team and community service agencies.
• Keep abreast of current trends in the field and integrate knowledge gained into current work practice, with support of Care Coordination leadership.
• Utilize motivational interviewing skills and culturally sensitive methods to collaborate with patients.
• Maintains high degree of communication with Care Coordinators and other clinical team members to assist in ongoing care coordination needs and communicate/remove barriers.
• Planning/Organizing/Initiating
• Participates and supports departmental quality improvement initiatives by sharing ideas and experiences and is willing to encourage others to do the same.
• Provide exemplary customer service skills both with patients, caregivers and community partners.

Qualifications

Education

• Bachelor’s degree in any field.
• In lieu of degree will accept 4 years customer service experience or experience in the insurance/social work/medical fields.

Experience

• Two years customer service experience or experience in the insurance field/social work/medical fields.

Licensure/Certification

• The American Heart Association’s Family and Friends CPR and/or Basic Life Support (BLS) training or retraining is required during orientation prior to providing patient care.
• CPR course and training must be maintained for the duration of employment.
