Job summary

Qualifications

• Demonstrated healthcare experience in a clinic or hospital setting.
• Demonstrated customer service with a focus on communications and problem resolution.
• Proficiency in advanced computer skills.
• Caregivers whose duties require home or community visits must maintain current BLS certification, have a valid driver’s license and insurance.

Responsibilities

• Acts as liaison between department and external organizations including payers, physicians, patients and caregivers.
• Transmits clinical information for service authorizations and coordinates with care managers and payers.
• Monitors referrals and maintains ongoing communication.
• Retrieves medical records for utilization review or quality assurance.
• Delivers regulatory notices to patients within required timeframes with proper documentation.
• Performs clerical tasks to support care management services, including scheduling and document handling.
• Supports advance care planning and notarizes documents on request.
• Assists in provider list maintenance and patient choice consultations.
• Coordinates with patients and families for transfers and community resource connections.
• Collaborates with care managers to ensure comprehensive and coordinated patient care.
