Job summary

Intermountain Health in Denver, Colorado is seeking a Care Management Assistant to coordinate patient care alongside nurses and social work care managers. The role acts as a bridge between patients, families, and the healthcare team, coordinating appointments and communicating with physicians.

You will educate patients about resources, support care management with regulatory and payer communications, and assist with documentation and transitions of care.

Qualifications

• Healthcare experience in a clinic or hospital setting.
• Customer service with focus on communications and problem resolution.
• Proficiency in advanced computer skills.
• Caregivers visiting home or community must maintain current BLS, driver’s license, auto insurance, driving record and reliable transportation.

Responsibilities

• Liaise between department and external organizations incl. payers, physicians, post-acute agencies, patients, and stakeholders.
• Transmit clinical information for service authorizations per contractual requirements and coordinate with payers.
• Monitor referral status and maintain ongoing communication.
• Retrieve medical records for retrospective utilization review or QA.
• Deliver regulatory notices within required timeframes with proper documentation.
