Job summary

Qualifications

• Knowledge of government and non-government payor practices, regulations, standards and reimbursement.
• Knowledge of Medicare benefits and insurance contracts/processes.
• Ability to demonstrate critical thinking, prioritization and time management skills.
• Must read, write and speak fluent English.
• Excellent interpersonal, verbal and written communication to collaborate with hospital leadership, physicians and payors.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitor patients’ stay for effective care coordination and resource utilization.
• Discharge planning with timely and safe transitions.
• Participate in interdisciplinary rounds to review goals and educate families.
• Collaborate with staff to develop and execute the plan of care.
• Refer high-risk patients for additional support and advocacy.
